Each Panther Creek is required to sign a Code of Ethics contract and turn it in to the Athletic Director (Coach Schuler) for each sports season. Please download the document, review it with your parents, and turn it in after you have signed it. No player is eligible to participate without completing this contract.

Critical Contact Information Form |

In addition to the WCPSS physical form, Panther Creek requires all athletes to complete a "Critical Contact Information" form that is kept in a binder by each coach. This binder is at practice daily and we take it to games so that we always have emergency contact information and a medical release for each player. Please download the form, complete it, and turn it in to Coach Schuler.
